How to Remove Yourself

The form requires you to select a reason from the following options:

Step 3: Provide Identifying Information

Step 4: Submit Supporting Documentation
Depending on your reason selected, you may be required to submit supporting documentation via attachment, fax, or postal mail. For example, identity theft victims may need to provide an FTC affidavit.

Important Notes
- SSN is optional but recommended for precise identification — without it, LexisNexis may not be able to identify and remove all records associated with you.
- Supporting documentation may be required depending on your reason — this adds processing time.
- Opting out may affect identity/insurance verification systems — LexisNexis provides identity verification services to banks and employers. Opting out of their people search does not affect this B2B service.
- Data does NOT repopulate once properly removed from LexisNexis consumer products.
